Allied Health Recovery Request Form

I'm on the bludge!!
Ignoring the fun we could have discussing how much someone got paid to develop the name of this form in such a positive, empowering light............Has anyone already converted this form to a word document or front desk document and would be prepared to share it as in the 'live' pdf document i can't import the frontdesk letter tags and I would like to be able to save it as a standard letter in frontdesk so this can occur.
Any copies from people who have already done this gratefully accepted at david.donkin@me.com
Thank you in advance

Hi David,
Just wondering how you got on with this? We've just had an insurer refuse our usual plans and instead want a AHRR completed and I can't integrate the pdf with Frontdesk.

Thanks David. Good to see the Community users helping eachother.

Adam, just to summarise, you need to create a standard letter (form) in Microsoft Word or the Front Desk Word Processor that will input the patient details into your required form.

Thanks steven. SIRA in their wisdom refused to convert the form to a Word doc for us.

Hi Justin

One of the benefits of the Community is to allow users to share information and assist each other. We have provided features in Front Desk to create standard letters and forms both in MS Word and the Front Desk Word Processor. We believe our time is better spent developing software functionality than implementing proprietary forms used by subgroups of Front Desk users.

It was very kind of David to convert the PDF form into a Front Desk usable format, thank you David.

Is it possible for David to add to the document to this conversation as there seems to be a few clinics interested in this.  Just to save him time in responding to all the requests. Sharon

Hi David

If you wouldn't mind opening up a support case so that I can contact you directly, I can grab the file and put it on our website. I will then send you the link so that you can insert it into a future post.

Hi all. Firstly, thank you David for assisting fellow users to date. Smartsoft has been working with SIRA over the last few weeks to develop the latest version (August 2016) of the Allied Health Recovery Request form in a Front Desk Document format. We are currently seeking approval from SIRA to make this document available to all users on the Community.

Hello Timothy;
I was wondering if there was any movement on the AHRR document with SIRA?  Obviously there are a lot of us hanging out for a Smartsoft compatible version!
Cheers!
Maryann

Hi Maryann. The form was submitted several weeks ago to SIRA for approval. We have queried SIRA several times for an update, but they are still in the process of reviewing the document. We will make the form available as soon as we have approval.

Hi all. We have received an update from SIRA that the form should be approved within the next 1-2 weeks. When we receive approval, we will provide further information on how the form can be downloaded and setup in Front Desk.

Hi Debi

This is a template specific for Front Desk. You may want to contact TM2 to see what they are doing in this area.
