Electronic Forms: new patient information

As one of our practices is paperless, we were hoping to have a simple new patients form available electronically ie. on a tablet whereby the information they enter is 'pulled through' to front desk. Similar functionality as the online booking request process only they're sitting in the waiting room and we could still do the final 'add' of the patient to ensure there's no duplicates.

Then we'd be pretty much 100% paperless

We are looking at doing the same. So if you go first, I would be really interested to hear how you go. Some of our clients would still need paper...

Excluding our new patient forms we too are paperless. I would welcome the addition of an electronic form as this would see us 100% paperless also.

Thank you for your input, this is the sort of valuable feedback we need.

Time to let you in on what we are already considering;
  • an electronic form on the booking gateway as part of the booking process that will put information directly into Front Desk.  We already collect patient name, address, DOB and contact number and move this directly into a patient file if booking as a new patient.  We need to add things like a consent for treatment, agreement regarding accounts etc and get them to sign the terms electronically.
  • if not using patient gateway, emailing a link to new clients so they can fill in these details at home or out and about on their mobile devices before arriving for their first visit.
  • in addition to the link above that could be sent in the practice at a first visit, if the client has a suitable mobile device with them to complete the form on their own device, a practice tablet (IPad) can be given to the patient to complete in the waiting room, which can also accept an electronic signature using finger signing technology.
In addition to how we deliver this, we also need to better understand what you want on your new patient (electronic) forms as this varies from practice to practice (and profession).

What details would you like on your electronic new patient information form ?

Hi,

This sounds like an awesome idea. Authority to release information would be good ie doctor, solicitor, school counsellor.

Also, would you be able to pick & choose what you had on your form? We have clients who are private, medicare, workcover, legal etc & we need different information from each different type.

Thanks

Anna

Hi Josie,

This would be a really good feature to front desk.

We do not use patient gateway but we would give the new patents an ipad upon they arrival to complete a new patient questionnaire. Would it be available in an app and able to use without gateway?

Have you looked at the Tonic for health app?
It looks awesome and so easy and fun to use for patients instead of just filling out a standard form. The format of the questionnaire is based on gaming graphics.
Obviously you could not replicate their app but if it was among the same lines it would be perfect!

Thanks

Sam

We would need to ask the patient their presenting complaint, pain scale, any previous care and complete a diagram/checklist referring to problem areas on the body.
Also, a main issue for us would be transferring the data collected to the patients file and saving this as an attachment.
We run FD though Hostus so hopefully we wouldn't have any issues because of this.

Hi Josie

Just like the clinical notes are, could this form or forms be customisable too?

I like the suggestions so far, including referral source and presenting complaint (what, how long, previous treatment). I would also like to add Medical History including medications.

Josie,
This would be extremely helpful if introduced. as well as the above mentioned info then claim details such as Insurer, claim number, date of injury, area of injury would be needed please - and if they automatically add to the Billing Details tab then happy days!

Yes, this would be awesome - is it still on the cards? Any ideas when it might be in place?

Hi Tonia. We agree, we think this is a great idea! I can confirm that it is on our development list and we aim to have this released in an update before the end of this year. We're constantly working on new features and improvements to Front Desk, it's just a matter of priority as to what ideas are implemented first. Follow this post to be kept in the loop with any updates.

Would be great but could we have additional text fields that we can name? Also if client is a child would need to be able to collect some info about the child as well as the 'linked' parent.

This reply was created from a merged topic originally titled new patient registration form.

Online new patient registration form that downloads straight into the database.  Our graphic designer says that our current pdf style forms are outdated and that most companies offer a form that the patient fills in online and submits.  The data gets populated on a database and is able ot be analysed and searched thorugh etc.  I am interested in this but am holding off in the hope that frontdesk is planning something similar in future.

Hi all. We are still in the early development phase for this feature, aiming for release in the first quarter of 2016. Rest assured we will be taking into consideration all of your suggestions throughout this process.

This is great news - if we could simply capture the patient's details as outlined in the "general" tab, that would be a great first step.  Also, if we could capture tracking / referral, that would be ace.

Hi Wendy. The previous time estimate was
provided in good faith based on the information available at the time. Software
development is a fluid process and at times we need to change direction in what
is important to be done now and what can wait. Our development team is always
working on the enhancement and improvement of the product as can be seen by the
regular updates. This item is one of the first things we will address
following the completion of the Xero integration. Thank you for the prod :)

Hi Tim,
I notice the Xero integration is in the works - will you also be developing a MYOB integration - that would save me a lot of data inputting!

Hi Andrew. Based on past experience, we are no longer providing exact release dates on new functionality as software development is a dynamic process. Things are always coming up that require us to reassess our priorities. Having said that, we are committed to this functionality and we believe it will be released this year.

Hi Mark. No further updates at this time as we are working on other priorities. An electronic intake form is still a high priority on our development list.

Hi Ross

Just to let you know, we have started this project. We have currently put it on pause as we are getting our head around the actual scope of what is required. As we speak to more users, it's clear that everyone has slightly different needs. We started off wanting to create a simple form that collected basic information and had the client give consent for treatment and take responsibility for their accounts, particularly if they were not paid by third parties. We have now moved on to being able to add a number of predefined questions from a pool of options.

At this stage, we are just determining what those questions should be. We have developed the code to allow users to mark a body chart and to electronically sign the form.

I have been waiting for this for two years and have begun to look at other options. I'd prefer you release a basic one just to input the first page of the client file and then make it fancier if you need to in an update. To put this into context I have to pay for an extra hour or two of admin support at $27 per hour EVERY DAY to enter this information from paper forms to front desk.., I don't even want to calculate what that looks like over the last two years but I'd be willing to pay to get an online form to do it to save me further costs!

i had liked Ross's comment above, because it indicated progress but I agree Gemille, the ability to at least enter basic patient data can be released and then the add-on components introduced at a later stage. Every practice will have differing needs, but the basic patient information from the General Tab would be vital for every practice i am sure.

Thanks

We would also really appreciate a simple electronic form, that at least filled the basic demographic details (copy the fields from frontdesk patient info?)
