As one of our practices is paperless, we were hoping to have a simple new patients form available electronically ie. on a tablet whereby the information they enter is 'pulled through' to front desk. Similar functionality as the online booking request process only they're sitting in the waiting room and we could still do the final 'add' of the patient to ensure there's no duplicates.
Then we'd be pretty much 100% paperless

Time to let you in on what we are already considering;
- an electronic form on the booking gateway as part of the booking process that will put information directly into Front Desk. We already collect patient name, address, DOB and contact number and move this directly into a patient file if booking as a new patient. We need to add things like a consent for treatment, agreement regarding accounts etc and get them to sign the terms electronically.
- if not using patient gateway, emailing a link to new clients so they can fill in these details at home or out and about on their mobile devices before arriving for their first visit.
- in addition to the link above that could be sent in the practice at a first visit, if the client has a suitable mobile device with them to complete the form on their own device, a practice tablet (IPad) can be given to the patient to complete in the waiting room, which can also accept an electronic signature using finger signing technology.
What details would you like on your electronic new patient information form ?

Hi,
This sounds like an awesome idea. Authority to release information would be good ie doctor, solicitor, school counsellor.
Also, would you be able to pick & choose what you had on your form? We have clients who are private, medicare, workcover, legal etc & we need different information from each different type.

This would be a really good feature to front desk.
We do not use patient gateway but we would give the new patents an ipad upon they arrival to complete a new patient questionnaire. Would it be available in an app and able to use without gateway?
Have you looked at the Tonic for health app?
It looks awesome and so easy and fun to use for patients instead of just filling out a standard form. The format of the questionnaire is based on gaming graphics.
Obviously you could not replicate their app but if it was among the same lines it would be perfect!

Also, a main issue for us would be transferring the data collected to the patients file and saving this as an attachment.
We run FD though Hostus so hopefully we wouldn't have any issues because of this.

This would be extremely helpful if introduced. as well as the above mentioned info then claim details such as Insurer, claim number, date of injury, area of injury would be needed please - and if they automatically add to the Billing Details tab then happy days!

Online new patient registration form that downloads straight into the database. Our graphic designer says that our current pdf style forms are outdated and that most companies offer a form that the patient fills in online and submits. The data gets populated on a database and is able ot be analysed and searched thorugh etc. I am interested in this but am holding off in the hope that frontdesk is planning something similar in future.

Just to let you know, we have started this project. We have currently put it on pause as we are getting our head around the actual scope of what is required. As we speak to more users, it's clear that everyone has slightly different needs. We started off wanting to create a simple form that collected basic information and had the client give consent for treatment and take responsibility for their accounts, particularly if they were not paid by third parties. We have now moved on to being able to add a number of predefined questions from a pool of options.
At this stage, we are just determining what those questions should be. We have developed the code to allow users to mark a body chart and to electronically sign the form.
