Hardware to write directly into Frontdesk software

Can anyone recommend some hardware where I can write directly into the Forntdesk note section via an electronic pen? eg. ipad or notebook. Can it be done? I'm looking to become fully electronic.

I am a psychologist and currently use Note Taker HD on an Ipad to write clinical notes which I then email as pdf to myself (from within Note taker). I then drop all pdf's into a sub-file called 'Client pdfs' within Frontdesk folder. From here I open client folder > Clinical Notes > New Note > Attach and attach the pdf to the client's clinical notes. I have all my files named "2016-09-07 - Surname First name" so they are easy to find. Works well for me but it would be good if somehow it could work straight to Frontdesk. I use a rubber nosed pen which wear out but I got onto some cheap (? $3-5) pens that last quite a while but they are nameless!
