The audit log is great, but has one potential flaw in the light of the increasing emphasis on privacy. Although it records when a clinical note is deleted, it does not show when|who accessed a  clinical note or when they are changed. Being able to keep a track of who accesses clinical notes would be great.

Hi Hari
to answer part of your post, if you make a change to a Clinical Note after the revision period (provided you have user permissions to do so), a revision is created keeping the original note intact.

You can select a modified note to see the change with the user who performed it and the exact time it was edited.
